- I have read the CDC's Activities and Initiatives Supporting the COVID-19 Response guidelines from May 20 and agree to observe those guidelines to all reasonable extents.
- Social distancing of at least six feet will be required and actively monitored and enforced.
- Employees are required to wear masks inside the business and when disinfecting surfaces and machines.
- Daily health checks of employees will be performed.
- Employees who are sick or are exhibiting symptoms will be required to stay home.

Extra Mile Enhanced Safety Restaurant Standards (select a minimum of 3)
- Schedule three or more times for disinfecting surfaces in high traffic areas.
- No self-service drink, utensil, or condiment stations.
- Train all employees on the importance and expectation of increased frequency of hand washing, the use of hand sanitizers and provide clear instructions to avoid touching hands to face.
- Require all employees to wear face coverings at all times. Such coverings shall be cleaned or replaced daily.
- Discourage workers from using other workers' phones, desks, offices, or other work tools and equipment.
- Where possible, stagger workstations to avoid employees standing adjacent to one another.
- Use disposable menus.
- Provide cafeteria-style salad bars and buffets.
- Thoroughly sanitize tables and any other commonly used items by guests.
- Use rolled silverware and no presetting of tables.
- Limit parties to no more than 10 people per table.
- Encourage reservation-only or call ahead reservations options.
- Post signs at the entrance stating no one is allowed in the restaurant who is exhibiting COVID-19 symptoms or a fever over 100.4.
- Use contactless pay options.
